摘要

本報告主要在描述如何照顧一位初次腹膜透析合併視網膜病變患者的護理經驗。護理期間為2012年6月9日至2012年7月20日。筆者在照護期間，經由觀察、電訪、家訪及應用Gordon十一項功能性健康型態評估等技巧收集資料，針對個案生活背景、教育程度及對疾病的認知做分析統整，發現個案有知識缺失、潛在危險性感染、焦慮等護理問題。藉由信任與護病關係的建立，依其個別性、獨特性，提供有計劃的衛生教育及相關護理措施，幫助個案能克服視網膜病變的限制，順利執行腹膜透析，而得以繼續維持以往的生活型態，面對未來的透析生活。

In this report, we describe the experience from initiating peritoneal dialysis in a patient with diabetic retinopathy. The data was collected from June 9 through to July 20, 2012. During the OPD visits, telephone interviews and home visits from this period, we applied Gordon's eleven functional health assessment techniques to assess the patient's background and his awareness of the disease for systematic analysis. Three health problems were found these being knowledge deficiency, risk of infection and anxiety. After establishing a nurse-patient relationship of trust, we provided personalized health education to help him overcome diabetic retinopathy and cope with a peritoneal dialysis dependent life style.
