本文描述一位54歲男性,因末期腎病變面臨血液透析無法預測及擔憂透析造成身體不適及生活上的改變,因而產生無望感之護理經驗。於2015年03月17日至03月31日,藉由整體性評估、觀察及深度訪談等方式,收集生理、心理、社會與靈性層面等資料,分析並歸納個案有體液容積過量、睡眠障礙及無望感等健康問題。以Watson關懷照護理論為基礎,經由限水、飲食衛教及血液透析治療,改善生理的不適;以同理心及支持的態度,透過陪伴,引導個案及家屬說出內心真實感受,並運用跨團隊腎臟個管師、營養師之介入,協助修正個案對水份的控制及飲食的認知,能以正向的態度面對透析治療,重新找回生命中的希望。期望藉此經驗的分享,作為往後照護此類病人的參考。
This article described a nursing care experience for a 54 years old male with end stage renal disease undergoing hemodialysis, who felt hopelessness due to worrying about the uncomfortableness and lifestyle change brought on by hemodialysis. Through March 17^(th) to 31^(th) of 2015, we used comprehensive evaluation, observation and in-depth interview to collect physical and psychologic data of this patient. We concluded that the patient had problems of excess fluid volume, sleep disturbance, and felt hopelessness. Based on Watson's caring theory, we introduced fluid restriction, diet education and hemodialysis to improve patient's physical condition. The caretaker's empathic and supportive attitude towards the patient allowed the patient and his family to gradually reveal his real feelings. Furthermore, through multidisciplinary cooperation and usage of available sources, we helped this patient face hemodialysis positively and regain his hope in life. We hope this special nursing experience could benefit nursing staff caring for similar cases in the future.
