某醫學中心精神科急性病房新型冠狀病毒肺炎之感染管制作為
Infection Control Procedures for COVID-19 in the Acute Psychiatric Ward: Experiences from a Medical Center in Taiwan
摘要
新型冠狀病毒肺炎疫情迄今影響全球醫療系統超過二年，隨病毒株不斷變異及高度傳染力，致我國發生多起醫療機構院內感染。精神科病房環境為封閉且病人認知理解力欠佳，執行防疫實屬極大考驗，故制定一套符合實務感染管制措施有其重要性。本文以臺灣某醫學中心急性精神科病房為例，透過跨領域團隊合作精神融入感染專科之護理思維，於單位內啟動「急性精神科病房－防疫應變小組」，並以加強工作人員及住院病人的健康管理、精神科病房感染管制作為為主軸，設計系列防疫作為，如：落實手部衛生與呼吸道衛生、設置工作人員個人防護裝備穿脫區域、環境清潔感染管制作業為核心，期望藉此經驗，為其他精神醫療機構傳染病感染管制指引。

Since two years, global medical systems have been impacted by the novel coronavirus (COVID-19) pandemic. During this period, the virus has undergone several mutations and become increasingly infectious, leading to multiple cases of nosocomial infections in medical institutions in Taiwan. Implementing COVID-19 prevention measures in psychiatric wards is a challenging task because of the closed environment of the ward and the compromised cognitive function of its patients. Therefore, there is an urgent need to develop a set of standard operating procedures suited to the current situation of the pandemic, especially focusing on infection control. In this pilot study, an Acute Psychiatric-Ward Pandemic Response Team was established based on interdisciplinary cooperation and the incorporation of the nursing standards of the infectious diseases control department in the acute psychiatric ward of a medical center in Taiwan. The team worked to enhance health management of the medical staff and inpatients in different dimensions, ensuring more precise infection control in the psychiatric ward and construing a series infection prevention procedures. The article describes implementing hand hygiene and respiratory hygiene, setting up areas for staff to put on and take off personal protective equipment, environmental cleaning, and infection control operations. It is hoped that these experiences could provide a direction for other psychiatric institutions that are still struggling to cope with the effects of the pandemic.
