Prevalence and Genotypes of Human Papillomavirus in Cervical Neoplasias

子宮頸腫瘤婦女感染人類乳突瘤病毒之基因型別

摘要


背景和目的:人類乳突瘤病毒(HPV)感染是婦女常見的性傳染病之一,研究發現長期持續性的感染是造成子宮頸癌的最主要原因。此研究之目的在於探討台灣地區子宮頸癌及癌前期病變組織中人類乳突瘤病毒之基因型別。方法:我們共收集了356位女性,組織學上確定診斷為尖形濕疣(10例),輕度鱗狀上皮細胞病變(78例),重度鱗狀上皮細胞病變(101例)及子宮頸癌(167例)來分析。為了增加實驗之準確度,本研究利用聚合?鏈反應技術來分析病毒的基因型。結果:在356位女性中,有343位(96.3%)發現有人類乳突瘤病毒的存在。在子宮頸癌的個案裡,以HPV 第16 型盛行率最高,其次為HPV第18,31,33及58型。但論及癌前期病變之病患,除了HPV第16,18,31及58型之外,HPV第52型亦極為常見。此外,HPV第16型之感染率隨著子宮頸腫瘤之惡性度有顯著上升之趨勢。結論:本研究結果顯示超過二十種人類乳突瘤病毒基因型感染和部份臺灣地區子宮頸腫瘤有關。本研究結果可作為臺灣地區預防子宮頸癌策略之參考。

Background and Purpose: A persistent infection with high-risk human papillomaviruses (HPVs) is associated with cervical carcinomas and their precursor lesions. The aim of this study was to explore the prevalence of type-specificHPV infections in precancerous lesions and invasive carcinomas of the uterine cervix in Taiwanese women. Methods: In our study, 10 condylomas, 78 low-grade squamous intraepithelial lesions (LSILs), 101 high-grade squamous intraepithelial lesions (HSILs), and 167 invasive cervical carcinomas (ICCs) were evaluated. To improve the detection and typing of HPVs in archival tissues, we conducted a comprehensive study in which PCR-based methods using E7 type-specific (MY11/GP6+) and L1 modified general primers (GP5+/GP6+) were employed. Results: Of the 356 cases, 343 (96.3%) were positive for HPV DNA. In this series, HPV-16, -18, -52, -58, and -31 were the 5 most common types in patients with LSILs or HSILs. In patients with ICCs, the 5 most common types were HPV-16, -18, -31, -33, and -58. The detection rate of HPV-16 increased as the severity of the cervical lesions increased. In contrast, HPV-52 and -58 were significantlymore prevalent in patients with LSILs orHSILs than in patients with ICCs. Conclusion: Our results demonstrated that more than 20 different genital HPV types are present in precancerous lesions and carcinomas of the uterine cervix in Taiwanese women. The prevalence of HPV-16 increased with the severity of cervical dysplasia. These data provide information for cervical cancer-prevention strategies in Taiwan.
