某區域教學醫院實施畢業後一般醫學訓練中社區醫學訓練之教學現況探討

Efficiency of Training in Community Medicine within a Post-Graduate Medical Training Program

摘要


背景與目的:探討畢業後一般醫學訓練計畫中社區醫學訓練的教學施行狀況及評估臨床教師與受訓醫師滿意情形,以做為日後訓練的改善方針。方法:以92年度在北部某區域醫院接受社區醫學訓練的第一年住院醫師和其臨床教師填寫「受訓醫師評量表」,「教師評量表」。結果:自92年10月至93年05月止,「教師評量表」實得有效問卷359份、「受訓醫師評量表」實得有效問卷447份。從「教師評量表」發現84.7%的臨床教師認為受訓醫師對於參與社區醫學訓練有興趣,僅有20.9%的臨床教師認為受訓醫師的社區醫學相關知識只維持在尚可甚至不足的程度。「受訓醫師評量表」發現只有17.4%的受訓醫師表示學習前一定會先行蒐集資料,19.2%的受訓醫師表示學習後會做詳細的教學記錄,但96.9%的受訓醫師覺得瞭解學習目標,91.5%的受訓醫師認為社區醫學的訓練對本身的臨床技能是有幫助的,說明了畢業後一般醫學訓練中社區醫學訓練的必要性。結論:受訓醫師對社區醫學訓練學習動機以及其社區醫學相關知識仍然不足,這是今後醫學教育仍需要加強的部份。

In this article, we explore the execution of and satisfaction with a community medicine training course within a post-graduate medical training program supported by the Taiwan Joint Commission on Hospital Accreditation in 2003. From October 2003 to May 2004, 359 Clinical Trainer Evaluation Sheets and 447 Trainee Doctor Evaluation Sheets were collected. Among trainers, 84.7% commented that the trainee doctors seemed to have interest in community medicine, while 20.9% claimed that the knowledge of trainee doctors was insufficient or barely enough. However, only 17.4% of the trainee doctors had collected any related materials before the training course began, and only 19.2% made a detailed summary after the training courses. Among the trainee doctors, 96.9% claimed to know the goal of this program, and 91.5% said the training course was useful in the development of clinical skills. This proves the importance of community medicine in a post-graduate medical training course. In the future, trainees' motivation toward and knowledge related to preclinical and clinical medical education need to be improved.
