Cerebral sparganosis is a rare parasitic infection caused by the larva of Spirometra mansonoides and is generally found in Asian countries. We report a case of sparganosis infection which presented as a cerebral tumor composed of intra- and extracranial components. The 2 components were connected to each other through skull defects caused by bone erosion. This 26-year-old man underwent surgical exploration for the mass. The pathological examination revealed the carcass of the worm with characteristics that suggested sparganum. We describe the unique appearances of magnetic resonance images and computed tomographic scans.
