Implementation of a Plan to Promote the Quality of primary Care in Shilin and Beitou District of Taipei City

摘要

本計畫之目的為提昇基層醫療品質，建立家庭醫師制度，提供全人醫療照護；社區醫院醫療資源開放，提昇醫療資源應用；醫療體系整合，落實雙向轉診制度，以提供民眾周全性、協調性與持續性的健康服務。期望能應用辦理基層醫師繼續教育、宣導民眾家庭醫師觀念、成立社區醫療群、開設共同照護門診、建立社區醫療健康資訊系統等方法達成目標。本計劃自九十二年八月十八日開始正式推行，至同年底為止，已轉診七十五病患，結果病患對陽明社區醫療群轉診的服務滿意度高達95%，雙向診比率達92%，健康家庭家戶檔案共完成1002戶，同意收案之家戶資料建檔管理比率達100%。雖然此項計畫普遍獲得社區民眾、基層醫師及社區醫院極高的肯定，但計畫內容非常耗費時間與人力，在講究績效的時代，若非院方與衛生局大力支持，恐怕很難永續經營，希望當局能有相關政策性的配合與實質的鼓勵，才能讓此有意義制度繼續下去，也才能因應當前民眾健康需求。

In order to promote the quality of primary medical care in Shilin and Beitou District of Taipei City, a plan was developed to organize the community family physicians, to provide whole- person medical services and qreater access to medical resources of community hospitals, to improve the efficiency of medical care system, to execute reciprocal transfer, transfer, and to provide comprehensive, coordinated and continuous medical care. To achieve these goals will require continuous education of general practitioners, convincing people of the importance of establishing care relationships with family physicians, organizing the community group medical practice system, common outpatient services, and improvement in community health information systems. This plan studied since August 18 of 2003, and seventy-five persons had received transfer services until December 31. The satisfaction rate with the transfer service provided was 95 percent; reciprocal transfer rate was 92 percent. A total of 1002 healthy family files had been established, and the completeness rate of data management was 100 percent.
