目的:探討臺北市性病患者的流行病學特徵及其盛行率。方法:研究對象是2002年7月至2003年11月間在台北市性病防治所(性防所)就診,且曾與個人史不清楚的對象發生性關係並在就診前一個月內未服用抗生素的初診患者。這些病患均探集其血液、尿液送檢,若為女性患者則加採子宮頸分泌物。檢體由性防所檢驗,檢驗內容包括愛滋病毒抗體、梅毒抗體檢驗,披衣菌、淋病病原菌檢驗,陰道鞭毛滴蟲培養。結果:2002年7月至2003年11月間共有282位符合條件的病患參加,其中男性230人(81.6%),女性52人(18.4%),平均年齡31.4±9.2歲,且55%受檢者是介於20-29歲未婚異性戀族群。受檢者中共有78位被檢出至少一種性病病原,其中檢出一種病原有59位(20.9%),同時檢出兩種性病病原有17位(6.0%),同時檢出三種性病病原有2位(0.7%),且78位感染者中有28位(35.9%)是無症狀感染。五種性病盛行率依次為披衣菌46人(16.3%),淋病26人(9.2%),梅毒抗體陽性20人(7.1%),愛滋病病毒5人(1.8%),陰道鞭毛滴蟲2人(0.7%)。結論:此次研究結果發現20-29歲的年輕、未婚、異性戀族群為感染性病高危險群。主要性傳染病病原依次為披衣菌、淋病、梅毒、愛滋病病毒、陰道鞭毛滴蟲,且一個人可能同時感染多種性病。本研究報告顯示了近年來台北市性病患者的流行病學特徵及其盛行率,可作為擬定防治政策的參考。

Objective: To assess the epidemiology and etiology of sexually transmitted disease (STD) in Taipei city. Methods: First-visit patients who had not taken any antibiotics were enrolled between July 2002 and November 2003 in the Taipei city STD Control Center. Blood, urine and cervical discharge samples were collected. Examinations included detection of Anti-H IV Ab, Anti-Treponema pallidum Ab, Chlamydia trachomatis, Neisseria gonorrhea and Trichomonas vaginalis. Results: A total of 282 patients including 230 males and 52 females were enrolled. Mean age was 31.4±9.2. Fifty-five percent were 20-29 years old and in the unmarried heterosexual group. We found 78 patients infected with at least one pathogen. Fifty-nine patients (20.9%) were infected with one pathogen, 17 patients (6.0%) with two pathogens, and 2 patients (0.7%) with three pathogens. Twenty-eight (35.9%) were asymptomatic. The prevalence of chlamydia, gonorrhea, syphilis, HIV and trichomoniasis were 16.3%, 9.2%, 7.1%, 1.8% and 0.7%, respectively. Conclusions: Young adults aged 20-29 years, those who were unmarried, and those who were heterosexuals were the groups at highest risk for STD. Chiamydia trachomatis, Neisseria gonorrhea, Treponema pallidum, H IV and Trichomonas vaginalis were the main pathogens among STD patients in Taipei City. This study describes the epidemiology and etiology of STD in Taipei city and could serve as a guide to the development of prevention strategies for STD. (Full text in Chinese)
