摘要


手多汗症在台灣有相當的盛行率,其為遺傳有關之疾病,手多汗症之致病原因目前尚未完全釐清,作者目前已累積了千餘例之經驗,發現手多汗症病患常合併家族史,因此應有特定遺傳基因型態。我們選擇與汗腺分泌有關之丁醯膽鹼基因來研究手多汗症病患之基因分布。本研究收集手多汗症病人58例,另有非手多汗症患54例,由白血球中萃取出DNA,利用聚合酶連鎖反應及RFLP技術得知這些遺傳基因因子之相關性及變異性。本研究中之目標爲丁醯膽鹼酶之K亞型變異性。結果於手多汗症病患中K亞型變異性之頻率爲0.1019,而非手汗症組中爲0.0648,兩組數據以卡方檢定求得P值爲0.2535,無統計上之差異。利用分子醫學技術將可能逐步篩尋出手多汗症之基因變異性。由於手多汗症在臺灣已被禁止施行於20歲以下的病人,對來的研究也勢必有一定的影響。

Backgrounds and Purspose: Among 50% patients with hyperhidrosis palmaris (HP), more than one family member in the fifth-degree relatives have concurrent problem of HP. Different racial preponderance has been reported in many series. It is thus postulated that specific genotypes are associated with HP. Sympathetic neurons Innervating sweat glands undergo a target-directed developmental switch in neurotransmitter properties. Most sympathetic end organs are adrenergic and functioning with norepinephrine as their neurotransmitter. The sweat glands are under control of the sympathetic nervous system; however their neurotransmitter is paradoxically acetylcholine. Butyrylcholinesterase (BuChE) is one of the key enzymes for degradation of acetylcholine in plasma. A genetic polymorphism of BuChE, K-variant, was associated with decease of enzymatic activity. The objective of this study is to investigate the correlation between K-variant of BuChE gene and HP. Methods: From 2000to 2002, 58 patients with HP and 54controls were recruited in this study. The K-variant allele was determined by polymerase chain reaction-restriction fragment length polymorphism (RCR-RFLP). Results: The allele frequencies of K-variant among HP and controls 0.1019 and 0.0648 respectively. K-variant of BuChE geng is not associated with HP, p=0.2535.
