摘要

目的：精神科住院常是處理自殺行為的治療模式之一。精神科住院病人的自殺雖不常見但卻對家屬及治療團隊成員造成重大的衝擊。本研究的目的即是分析住院中自殺成功的精神科病人的臨床特徵，以了解可能的相關危險因子。方法：某一醫學中心的精神科病房，於十年內有八位自殺成功的住院病人。以病歷回溯的方式，分析其流行病學資料及臨床特徵，以了解自殺事件發生前後的各項狀況。結果：部分特徵與其他國內外的研究結果一致，包括：所有病人均符合情感性疾患診斷，住院時均呈現憂鬱與激動焦慮情緒，且住院時主訴均是有自殺意念或行為。結論：本研究發現三項臨床上重要現象：一、所有病人均是因病情復發於三個月內再次住院；二、自殺發生前，部分個案經歷病情起伏，甚至有人呈現病情好轉的現象；三、多數自殺事件發生在週末及週一。住院中病人的自殺防治是臨床上非常重要的課題。

Objectives: Psychiatric hospitalization is one of the treatment methods for severe suicide attempts. Psychiatric in-patient suicide is thought of as rare but shocking to the patients' families and heath care professionals. This study aimed to analyze the clinical characteristics of committing suicide in psychiatric in-patient units, in order to understand the possible associated risk factors. Methods: In a ten-year period there were 8 psychiatric in-patients who committed suicide within the psychiatric units of a medical center. The authors reviewed the socio-demographic and clinical data of all 8 cases, according to the chart records, to analyze the related circumstances surrounding each suicide event. Results: Some of our study results were consistent with the findings of other studies; all patients were diagnosed with mood disorders, suffered from depressed and agitated moods and were hospitalized for treatment due to suicide risk. Conclusion: The following three common conditions were found amongst our subjects. 1) Re-hospitalization within three months after symptom relapse. 2) A fluctuating even improving clinical course prior to the time of suicide. 3) A large proportion of suicide attempts occurring on weekends or Mondays. The clinical significance of these factors needs further examination in a future study.
