連結臨床電子病歷與實證醫學資料庫之研究

摘要


對於醫療人員來說醫學文獻的取得,一直都是很重要的一個課題,其中更以醫師為甚,醫師終日需面對許多臨床上不確定性的問題,因此醫師需要不斷增進自我的臨床知識,實證醫學(Evidence-based Medicine)即是以應用於臨床工作的角度為重點,運用可用資訊中最好的方法來增進病人之健康,為了讓醫師簡單獲得實證醫學文獻,許多研究者便去開發特定的系統來解決這方面的問題,資訊按鈕(Infobuttons)即是一例來解決此類問題,目標在於協助醫師面對於病人資料時,能夠透過一個介面將病人的病症轉換成醫師有興趣的關鍵字,再以這些關鍵字去搜尋後端數位圖書館目前最新的文獻,並透過資訊檢索(Informational Retrieval)的技術,得到醫師想要的文章內容;本研究將開發整合電子病歷之系統,並提供API(Application Platform Interface)界面之功能,提供系統上使用的彈性,將病人的診斷視為查詢實證醫學之關鍵字,由關鍵字(key word)直接連結至實證醫學資料庫(evidence-based database),協助醫師獲得所需之醫療文獻。

Background and Purpose: In performing medical research, it is important to examine medical reference materials, especially when assisting physicians to address indeterminate clinical diagnoses. It is important for researchers and physicians alike, to continuously update their understanding of clinical information so that they may provide current medical consultations. Methods: To better assist medical doctors search evidence-based medical reference material, many researchers develop special systems to address commonly reported problems. 'Info buttons' are an example of such purpose-built search tools. This system translates patients' symptoms into certain medical key words. By searching key words, highlighted in recent literature stored in digital libraries, librarians can apply information retrieval techniques, allowing doctors to access the most up-do-date medical references to help complete diagnosis. Results: The Taipei City Hospital is developing a system to integrate digital medical records with an Application Platform Interface (API). This system links key words from patients' diagnoses directly to an evidence-based database, thus helping medical doctors to make better informed decisions. Conclusion: We developed the integrated environment which combined the electronic medical record system, and offered API (Application Platform Interface) functions of the interface in our research, not only offered the elasticity used on the system, but also regarded the patient's diagnosis as the key word which connected to Evidenced-based medical database. With this operation mode, the system helped physician got applicable medical literatures.
