Removal of Floating Foreign Body in the Urinary Bladder with Carbon Dioxide Infusion

用二氧化碳灌注膀胱以取出漂浮於膀胱內的異物

摘要

我們報告一個病例，一位21歲男性病人把一段圓柱形塑膠套管由尿道塞入膀胱，卻無法解出。膀胱鏡發現該塑膠套管漂浮在膀胱頂部，我們累了一個多鐘頭還抓不住那段小塑膠套管，因為它很滑又在膀胱頂部漂來漂去，異物鉗一夾，它就滑開。後來我們改用二氧化碳氣體灌入膀胱，讓塑膠套管掉在膀胱底部，不再亂漂。異物鉗就能將塑膠套管壓在膀胱壁上，穩穩地抓牢它。不到五分鐘，就像夾出小石頭般順利夾出塑膠套管。

We present a 21-year-old male patient who inserted a plastic plug into his bladder but he was unable to void it out. Cystoscopy found the plug floating at the dome portion of the bladder. We tried for more than 1 hour but failed to remove the plug. Because the plug is round and slippery we could not grasp it firmly. It floated away swiftly on touch of the foreign body forceps. We replaced the infused isotonic sodium chloride with carbon dioxide to keep the plug at the base of the bladder. Then using foreign body forceps we pressed the plug against the bladder wall and grasped it firmly. In less then 5 minutes the plug was removed as easily as a small bladder stone.
