摘要

臺北市於後煞時期積極建構系統性感控防疫架構，將各家市立醫院整併為臺北市立聯合醫院，全力建構統合性資訊平臺及培訓應變式人力資源，其下設置傳染病防治部及疾病管制院區以專責防疫公衛任務。因應H5N1流感大流行之整備除針對境外、社區、醫院等節點進行監測，並建構「看診三問」及「急診自動化症候群監測系統」以早期發現個案攔截病毒。當首例H5N1流感入侵北市將動員本院約200名機動防疫隊進行疫調以嘗試封鎖圍堵疫情，並啟動和平院區感染症專責醫院，其餘院區則開設戶外疫病篩檢站及留置病房之動線管制。一但疫情進入群聚之大規模流行，在總體醫療分流之原則下本院將依序啟動備援專責院區，並配合市府設計之大型收治中心組成流感專責醫療團隊。其他未被徵收之院區則積極進行醫院之感控保全，並負責基本社區醫療以充分發揮臺北市立聯合醫院的公衛照護任務。

In post-SARS era Taipei City Government has implemented an integrated system and strategy against emerging contagious diseases. Ten municipal hospitals had been transformed into one union Taipei City Hospital that includes Department of Contagious Disease Prevention and Branch for Disease Control and Prevention. In preparing for pandemic H5N1 influenza, an onsite hospital-wide active surveillance system by using TOCC questionnaire and chiefcomplaint based automatic syndromic surveillance system were applied in order to spot H5N1 virus at first place. As soon as the case of H5N1 influenza human transmissions in Taipei occurs, the strategy is to activate some 200 members of the prompt response epidemiological investigation team to contain the disease within special isolation hospital of Hoping Branch. The same time, outdoor screening stations and detention wards will be set up at other branches of the Hospital as additions to a strict infection control programs. If there are clusters of outbreaks in the community, the next 2 pre-selected branches and schools will be transformed into special isolation hospitals and shelters, respectively, to accommodate medical care for the large scale of casualties. The rest of the branches of Taipei City Hospital will remain as general-care community hospital in order to maintain the fundamental structure of social security and basal medical requirement. Through the preparedness plan, a well-designed information system and surge capacity in human resource are also in place. It will consolidate and enhance the capability of Taipei City Hospital to archive the mission in public health care.
