以足底壓力變化來探討針刺對眩暈患者之療效

摘要

眩暈患者常以頭暈目眩、站不穩或走路會有跌倒的感覺作為主訴來就診。內耳前庭性眩暈屬周邊性，發生原因多和病原性及病理性有關，常引起多重感覺異常及造成感覺運動機能的障礙。足底壓力板(footscan)可以客觀記錄足底壓力中心(COP)的變化，來反推身體重心的改變，故可以探討受測者於動態行走或靜態站立的重心變化。臨床上，針刺身體上的某些穴位可以治療及改善眩暈的症狀，因此對眩暈患者的站姿平衡控制應有一定程度的改善。受測者分為眩暈組及正常對照組，眩暈組再分為實驗組（針灸針刺組）及對照組（磁珠貼穴組）。每一受測者以睜眼、閉眼，配合雙腳站、雙腳前後站（慣用腳前、另一腳後）以及單腳站（左腳及右腳）等八種不同的靜態站立姿勢。結果在各種站姿下•眩暈患者的總站立時間除了睜眼雙腳站、閉眼雙腳站及睜眼前後站與正常人一樣，其他站姿的總站立時間較正常短。在其他的站姿下，眩暈患者的COP總位移路徑較長、COP平均移動速率以及平衡狀態下之COP橢圓面積較正常人大。在治療前後的比較上，針刺組在不同站姿的參數都有改善的趨勢，且優於對照組，尤其在閉眼前後站的姿勢下，出現較多的顯著差異。

Vertigo patients often complain of dizziness and feeling faint throughout the day. Clinically, there are two types of vertigo; the central type and the peripheral type. Vestibular vertigo, belonging to peripheral category, is found to be caused by etiology or pathology diseases. We can objectively record the changes of the center of pressure (COP), using the footscan platform. The record also gives us awareness as to the changes of the center of body mass. It is found that by needling some acupoints on the body, the balance control ability of these patients may be improved. It is therefore stated that acupuncture aides in the treatment of the vertigo disease. The vertigo subjects used were divided into two groups. The ACU group, using the acupuncture treatments, and the control group without the use of acupuncture. We found normal subjects to be the contrast group. Each group was asked to perform the two foot stance, the tandem stance, and the single foot stance. All of these were doing with the eyes open, and then again with the eyes closed. There were some significant differences between vertigo and normal subjects. For example, vertigo subjects had shorter total standing time, longer COP traveled way, faster COP mean velocity and larger elliptic areas of COP during standing than normal subjects. After treatments, the ACU group showed more improvement than control group. And there were some significant differences in elliptic areas during tandem stance with eyes open and eyes closed. The significant difference also showed in the total COP travel way during the single left foot stance with eyes open.
