客觀結構式臨床測試之規劃及執行

Designs and Applications of Objective Structured Clinical Examination

摘要


截至目前為止,要對學員評量其臨床能力中的「表現怎麼做」也就是評量學員的「核心技能」,是觀察學員直接對病人施做「核心技能」時的表現。但為顧及病人安全、為讓學員能充分學習及接受完整評量,以客觀結構式臨床測試最佳。因為此測試法不直接做在病人身體,是可以兼顧「以病人為中心」及「以學習者為中心」雙方。客觀結構式臨床測試可用來評估美國畢業後醫學教育評鑑委員會對學員要求的所有六項核心能力,而且設計是標準化,以客觀、結構式來評量,對同儕間比較是較能達到公平的。。本文論述以客觀結構式臨床測試評量學員6項核心能力時之規劃及執行。

Up to the present time, the task of assessing ”clinical skill” in a medical learner involves the individual ”showing how” something is done and consists of observing the learner's performance with patients. In this context, the objective structured clinical examination (OSCE) has become the method of choice based on patient safety, measuring complete learning and carrying out a full assessment. Moreover, OSCE is able to assess all six of the general competencies established by Accreditation Council for Graduate Medical Education. The OSCE is standardized, designed with an objective in mind and consists of structured concepts that allow a fair evaluation of the learner. This article describes the planning and application of OSCEs.
