原發於外聽道的惡性腫瘤相當的少見,每年發生率約為百萬分之一而且超過80%的外聽道的惡性腫瘤是屬於鱗狀上皮細胞癌(squamous cell carcinoma),至於腺樣囊狀癌(adenoidcystic carcinoma)更是少見約佔5%[1]。腺樣囊狀癌的復發率很高。一般而言,耳部的腺樣囊狀癌是由汗腺體(ceruminous glands)產生,易有局部軟組織及骨頭的侵犯[2]。本院於2004年10月經歷1名47歲女性病患,其主訴為左耳耳痛約2週,耳鏡檢查其耳膜及外耳道為正常,但在耳廓中間處有一小腫塊約0.5公分。於是安排患者進行切除手術。目前患者追蹤5年,至今未有局部復發或轉移的情形,因病例罕見,故提出報告。
Primary external canal carcinoma is rarely found in the external auditory canal. The incidence is approximately one out of a million people per year. We have reviewed the previous literature and found that more than eighty percent of primary malignant neoplasms found in the external auditory canal are squamous cell carcinoma. Primary adenoid cystic carcinoma accounts for only about five percent. Generally, adenoid cystic carcinoma is thought to arise from ceruminous glands and seems to easily invade surrounding soft tissue and bone. The high recurrent rate of ACC had been discussed in the literature. In this context, we repor t a 47-year-old woman who initially presented at the outpatient clinic of the Department of Otolaryngology at Changhua Christian Hospital with left auricular pain for two weeks. An otoscopic survey revealed a normal eardrum, but the presence of a mass, about 0.5 cm at its greatest diameter, was noted on her left auricle. An excision of this left auricular tumor was then arranged. The pathological result showed that this was an adenoid cystic carcinoma. The patient has been followed-up for five years after the surgery with no recurrence up to the present.
