臺北市立聯合醫院全責照顧計畫之滿意度探討

Satisfaction with the Skill Mix Nursing Model in Taipei City Hospital

摘要


目的:1.探討實施全責照護制度,對改善陪病率之成效;2.探討病人/家屬對全責照護制度之實施,對病房助理員協助照護工作之滿意度;3.探討護理人員對全責照護制度之實施,對病房助理員協助照護工作之滿意度;4.探討病房助理員對全責照護制度之實施,自我工作滿意度。方法:以本院五家院區、護理部所屬護理單位之內、外科病房、護理之家、呼吸照護病房及1個精神科加護病房,總計48個實施全責照護制度單位之病人/家屬、護理人員、病房助理員為研究對象母群體,採方便取樣。使用自擬問卷收集樣本基本屬性、病人/家屬及護理人員全責照護制度,及病房助理員滿意度。統計方法包括平均數、標準差、百分比描述基本資料,及滿意度。結果:1.總體陪病率顯著下降至21.03%;2.病人/家屬對病房助理員執行輔助照顧活動,滿意度達94.44%;3.護理人員對全責照顧制度整體滿意度平均得分82.45(SD=10.89);4.病房助理員自我工作滿意度平均得分57.06(SD=6.28),對工作互動關係普遍感到最滿意。結論:本研究發現病房助理員協助護理作業,使護理時數應用於直接照護病人、維持護理品質水準,病人/家屬的照護需求能被滿足,病房助理成就感提升、繼續留任。

Objective: 1.Demonstrate the effectiveness of skill mix nursing model on decreasing bedside companionship; 2.Explore the satisfaction of patients/ family members to the application of skill mix nursing model and the service of ward assistants; 3. Explore the satisfaction of nurses with the application of skill mix nursing model and the service of ward assistants; 4. Explore the satisfaction of ward assistants with the application of skill mix nursing model and the service provided by themselves. Methods: Patients/family members, nurses and ward assistants from the internal medical wards, surgical wards, nursing home, respiratory care wards and psychiatric ICU under the Department of Nursing in 5 of 10 hospitals, 48 care units in total, were included as the study, and a convenience sampling was used. Data were collection such as the satisfaction of patients/family members, satisfaction of nurses with the application of skill mix of nursing model, and the job satisfaction of ward assistants. Descriptive statistical analysis was obtained as mean, standard deviation, and percentage. Results: 1.The overall bedside companionship dropped significantly to 21.03%; 2.The satisfaction rate of patients/family members with the aided care provided by ward assistants was 94.44%; 3.The mean of overall ratings on satisfaction of the application of total care rated by nurses was 82.45 (SD=10.89); 4.Ward assistants' own ratings for mean job satisfaction was 57.06 (SD=6.28). Conclusion: This study found that having ward assistants assist with care tasks allowed nursing hours applied directly to patient care needs. The ward assistants in turn experienced a greater sense of job satisfaction and willingness to retention.
