提升安寧緩和醫療病房足部護理作業之正確率

Improving Foot Care Practice at Hospices and Palliative Care Units

摘要


癌症高居十大死因之首,癌末病人因病情變化或治療因素,常導致淋巴水腫症狀,藉由美足護理能緩解症狀、促進舒適,以滿足緩和醫療病房住院病人照護需求。本專案旨在提升緩和醫療病房護理人員執行美足護理的正確性,利用病歷回溯法收集資料及設計問卷進行現況調查,發現造成美足護理不正確原因為缺乏美足護理技術標準、美足護理認知偏低、教育訓練時數不足。改善措施包括設立美足護理標準作業流程、增加在職教育次數、美足護理教學指南及進行定期技術稽核。結果顯示:護理人員對美足護理認知正確率由47.3% 提升至100%,及執行美足護理正確率由54.5% 提升至95.6%。經由此專案改善,不僅有效提升執行美足護理的正確性,進而提升照護品質。

Cancer is first among the ten leading causes of death and because terminal cancer patients often undergo changes in their condition and/or treatment, this may often lead to the symptoms of lymphedema. By treating feet as part of nursing care, this will relieve symptoms, promote comfort and help to meet the needs of hospice and palliative care unit patients. The aim of this project was to improve foot care at hospices and palliative care units. A chart review was used to collect information and a questionnaire was designed to collect information; these approaches were then used to assess the current situation. A failure to carry out appropriate foot care was related to a lack of standard procedures, a low level of knowledge, a lack of education in the area and limited training in the area. Improvements to foot care were brought about by establishing standard procedures, by increasing education in this area, by providing teaching guides and by carrying out a regular technical audit. The changes resulted in the understanding of appropriate foot care being increased from 47.3% and 100% and the application of appropriate foot care being increased from 54.5% to 95.6%. This project effectively improved foot care in hospices and palliative care units, thereby enhancing the quality of care of these terminally ill patients.
