下泌尿道症狀對於女性性功能之影響

The Impact of Lower Urinary Tract Symptoms on Female Sexual Function

目的:過去研究多針對女性尿失禁患者的性功能,極少著墨於其他下泌尿道症狀(Lower Urinary Tract Symptom, LUTS)患者。我們用OABSS(Overactive Bladder Symptom Score, OABSS)、AUA-SI(American Urological Association Symptom Index, AUA-SI)和FSFI(Female Sexual Function Index, FSFI)三種評估工具,研究LUTS對女性性功能之衝擊。方法:健康女性醫院員工參與此前瞻性研究,分析其人口屬性、OABSS、AUA-SI、FSFI相互關聯。此外,受測者分OAB(OABSS ≧ 3)與非OAB,輕與中至重度LUTS(AUA-SI ≧ 8),比較其FSFI。結果:89位受測者平均年齡43.31±8.87。OABSS平均2.34±2.09,AUA-SI為3.34±4.31,FSFI為21.53±10.19。其中OABSS與FSFI的性興奮、陰道潮濕、性高潮、疼痛面向呈負相關(r分別為-0.235、-0.212、-0.250、-0.212,所有P<0.05)。AUA-SI與FSFI的性興奮、陰道潮濕、高潮、滿意度面向呈負相關(r分別為-0.264、-0.222、-0.224、-0.210,所有P<0.05)。LUTS較嚴重者其FSFI總分顯著較低,且表現在性興奮、陰道潮濕、高潮、滿意度(P<0.05)。結論:女性性功能受到LUTS的負面影響,影響最嚴重的面向是性興奮、陰道潮濕、高潮和滿意度。

Objectives: Previous studies have investigated the female sexual dysfunction (FSD) among women with urinary incontinence, but few have studied the sexual function in women with lower urinary tract symptom (LUTS). We define the impact of LUTS on FSD using three validated instruments, the Overactive Bladder Symptom Score (OABSS), American Urological Association Symptom Index (AUA-SI) and Female Sexual Function Index (FSFI). Methods: Female hospital employee were enrolled prospectively. Past medical history was reviewed. The correlations between OABSS, AUA-SI total and subscores with six domains and total FSFI scores were analyzed. In addition, subjects were categorized into OAB (OABSS ≧ 3) or non-OAB, mild LUTS or moderate to severe LUTS (AUA-SI ≧ 8) for further comparison of their FSFI scores. Results: 89 subjects with mean age 43.31 (range 30-66) were enrolled. The mean scores in OABSS, AUA-SI and FSFI were 2.34 (range 0-10), 3.34 (range 0-20) and 21.53 (range 2-34.2) respectively. The OABSS scores were negatively correlated with FSFI total scores and domains in arousal, lubrication, orgasm and pain (r=-0.235,-0.212,-0.250,-0.212 respectively, all p<0.05). The AUA-SI scores were also negatively correlated with FSFI total scores and domains in arousal, lubrication, orgasm and satisfaction (r=-0.264, -0.222, -0.224, -0.210 respectively, all p<0.05). Moreover, women with moderate or severe LUTS had significantly lower scores in FSFI total score and domains in arousal, lubrication, orgasm and satisfaction (all p < 0.05). Conclusion: The sexual function of women is negatively influenced by the presence of LUTS. The sexual domains most affected are arousal, lubrication, orgasm and sexual satisfaction.
