加味黃連解毒湯治療妊娠濕疹－病例報告

一位32歲女性，自妊娠五個月起，胸部及下緣開始發癢，初起時塗抹西藥藥膏後搔癢減緩。但一週後全身起紅斑，發癢嚴重，搔抓後起紅色丘疹，且病灶逐漸漫延，遍生於胸、腹、背部及四肢，除了頭面及手足掌心外，全身皆作，甚至抓到滲血，且紅疹不退。經西藥Cetia^®、Chemin^®軟膏、金貝比及中藥消風散等治療後，病況僅約改善1-2成，仍然搔癢嚴重，導致情緒煩躁，且夜間反覆癢醒影響睡眠，故至本院中醫科就診。來診時發現個案平素甚畏熱，喜食重鹹、炸、辣，易便秘；患處呈現丘疹及紅斑，色澤焮紅，夾有血痂抓痕，遇熱則發癢加重；舌色紅，有朱點，苔薄白，脈弦滑數。證屬血熱生風，故以清熱涼血，袪風止癢為治則，處方用黃連解毒湯加味治療。連續服用一個半月後皮膚病灶完全消退，治療成效良好，顯示以中藥治療妊娠濕疹是不錯的選擇。

A 32 year-old female has suffered from skin itching over chest wall since the fifth-month of gestation. The symptoms were mildly relieved after ointment applied. Nonetheless, one week later, general itching was noted, and the lesions of erythematous plaques were dispersed on the chest, abdomen, back, and extremities, sparing the head, face, palms and soles. Additionally, the lesions were excoriated and erythematous papules were found after scratching. After receiving western and Chinese medical treatments, however, the symptoms persisted, resulting in poor sleeping condition and impatience. The patient visited the out-patientdepartment of Chinese Medicine of our hospital. We noticed that her dietary habit was salty, fried, and spicy. She was sensitive to heat and had constipation. The itching aggravated under higher temperature. Her differentiated pattern is hot blood causing wind. The treatment principle for such pattern is to eliminate the heat, and expel the wind to relieve itching. Her prescribed formula was the modified Huang-Lian-Jie- Du decoction. After one and a half month treatment, the itching skin lesions completely subsided, and the treatment result was satisfied.
