Objective: We conducted an anonymous feedback analysis of the team members of the cross-department care team on home visit services, as a reference for next stage services. Methods: this was a qualitative study. The data were the self-assessments of the members of the cross-department care team. Results: Among the elements of good health outcomes, health care organizations and design for health service delivery were concerned good. However, community resources, community policy and clinical decision support, information systems, and public self-management were still to be improved. Conclusion: The feedback analysis of the cross-department care team has established a new service model and may be referred in future policy planning.
