摘要

目的：院區醫護藥理社工團隊結合社區里鄰長志工團隊，提供社區民眾整合性的照護。方法：每星期院區組織醫護藥理社工團隊到社區，協同社區里鄰長志工團隊針對需要關懷的社區民眾進行家庭訪視，說明照護內容，簽署同意書，評估照護個案身體狀況，提供照護建言。結果：自104 年7 月22 日到12 月30 日止，共計出訪22 次，收案照護76 人。協助10 位民眾到院區就醫76 診次，其中以家醫科、牙科和中醫科最多。有44 位收案民眾有填寫滿意度調查，全部回答「非常滿意」。結論：由院區醫護藥理社工團隊協同社區里鄰長志工團隊到宅訪視民眾收案照護，並協助有需求民眾到院就醫，獲得民眾高度肯定。

Objective: This study examined the public satisfaction on community based integrated cared program which combined the efforts of hospital health care team volunteers of neighborhood leaders. Method: Every week the hospital health care team, together with the neighborhood leaders and volunteers, went to the community for home visiting, explained the care content, signed the consent, assessed the cases, and gave health care advice to people who needed. Results: The study duration was from Jul. 22, 2015 to Dec. 30, 2015. The team visited the community 22 times and a total of 76 clients were included. 10 clients were arranged to our out-patient clinic for 76 person-visit, mostly the Family Medicine department, Dental department and Chinese Medicine department. Among the clients, 44 accomplished the satisfaction Questionnaire, and all responses were ＂very satisfied＂. Conclusion: The hospital health care team supported by community leaders and volunteers went to the community for home-visit and provided assistance for medical treatments. The service was highly recognized by public.
