Developing a Palliative Care Service-Learning Course for Medical Students

摘要


近年來安寧療護蓬勃發展,臺灣安寧療護照護品質排名高居全球第6 位,然而,在求學過程的醫學人文課程裡,教導醫學生如何預防罹患不可治癒疾病的民眾受苦的課程較少。本課程以建立醫學人文關懷為核心設計理念,核心課程中邀請具臨床實務經驗的核心教師,在學生實際參與服務之前給與核心內容學習,讓臺大醫學生對安寧療護服務內容具備應有的知識。服務課程中包含安寧病房介紹、醫學生實際參與床邊服務、查房、病房會議、家庭會議參與、傾聽陪伴服務—傾聽、精油按摩、安寧共照參與及倫理議題探討與社區安寧服務等。學生參與課程後的滿意度達100%,本課程希望能拋磚引玉,期許安寧緩和醫療、社區醫療的觀念能夠持續向全國醫學生推廣,並讓此課程計畫延續。

Despite the broad support for palliative medicine education and training in hospitals in Taiwan, there is still a lack of related curriculums in medical schools, especially for preclinical students. In 2015, supported by the Taipei City Hospital, we created a student-centered course for palliative care for medical students from the National Taiwan University School of Medicine. The students were divided into 4 groups with the largest group consisting 5 members, and each group was assigned to a different branch of the Taipei City Hospital. This 40-hour course introduced palliative care through three stages: classroom curriculum, clinical observership in hospice ward, and clinical observership at patient’s home. First, students learned the basic concepts of palliative medicine and home-based holistic care. Second, they began clinical observership in a hospice ward involving interaction with hospice patients. Finally, led by medical teams, the students visited patients in the community. The curriculum had a 100% course satisfaction in all three stages, and all 16 preclinical students believe that understanding palliative care and home-based holistic care are helpful in their future clinical training. The curriculum has become a formal service learning course in the National Taiwan University School of Medicine and the 2nd program for new first grade medical students launched in 2016.
