摘要

用藥安全是醫療品質重要的一環，1990 年Hepler CD 和Strand LM兩位學者倡導推展藥事照護(pharmaceutical care)，其定義為藥師有責任提供與藥物有關的照護，讓病人達到一個明確的治療目標，進而提升病人的生活品質；2007 年3 月21 日我國新修正的藥師法：「藥事照護相關業務」，開啟了藥師直接照顧民眾藥物治療的專業功能。結合公共衛生、醫療體系及社區關懷體系，將醫院的功能主動化，藥劑科配合家庭醫師責任制將醫療服務直接送入市民家中，本文的案例為75 歲病人，有高血壓和糖尿病，雙目失明且重聽，長期往返榮總和萬芳2 間醫院看診；經由醫療團隊的探訪，願意接受居家醫療計畫的服務改至仁愛院區家醫科與新陳代謝科就診。瞭解病人的需求，不只是幫助病人治療疾病，也瞭解藥物治療過程中正確的專業知識，病人藥物儲存恰當與正確的藥事服務觀念，進一步能夠教導病人或家屬提升生活品質。

Drugs safety is the key to medical quality. Hepler CD and Strand LM (1990) advocated and promoted pharmaceutical care in which the pharmacists are rendered the responsibility to provide drug related careto reach a clear treatment goal and improve the quality of life of the patients. On March 21,2007, regulations on pharmaceutical care related business were amended to the Pharmacist Act. This initiated the professional function of pharmacists to take care of patients directly. With the integration of public health service, medical system and community care system, the hospital has taken medical services initiatively, and the pharmacy delivers its service directly to homes in in coordination with the Family Physician Accountability Project. The case was 75 years old patient. He suffered from hypertension and diabetes for many years, blindand with hearing impaired. He visited outpatient departments at the Taipei Veterans General Hospital and Wanfang Hospital for a long time. After the visit of our medical team, he was willing to accept the integrated medical project at home provided by the family division and metabolism division of Ren-Ai branch. Not only helping patients to cure their illnesses, we also understand patients’ need and deliver correct knowledge of pharmaceutical treatments, methods of drug storage, and the right concept of pharmaceutical care, and educate the patients and their family members to improve quality of life further.
