居家醫事檢驗照護服務模式之建立
Establishing an In-Home Model of Medical Laboratory Services
摘要
我國65 歲以上老年人口預估120 年快速增加至120 萬，因此如何落實全人醫療、強化居家醫療的服務品質日趨重要，然醫檢師在居家醫療的角色與任務未定，北市聯醫之醫檢師自105年3 月1 日居家整合醫療計畫開始至7 月底共參與30 位居家照顧服務及8 位安寧療護個案，其中抽血服務佔66.7%，血糖機檢測6.7%，心電圖檢驗3.3%，其餘為配合團隊量血壓、關懷、協助病歷製作、紀錄相關資料等。由訪視經驗顯示抽血服務為居家醫事檢驗首要任務，統計居家訪視所有檢驗項目發現所需配合檢驗的重點為血糖及肝、腎功能、膽固醇及常規血液檢查，將來預計配合醫療端之需求推動攜帶型心電圖檢驗、檢驗諮詢、快篩檢驗及POCT (point ofcare testing) 檢驗。快篩檢驗及POCT 檢驗最快幾秒鐘最慢20 幾分鐘內可獲得即時檢驗結果，醫檢師日常依標準手冊維護以保障檢驗品質，亦可教導其他醫事人員正確之使用方式，以期更利於居家整合醫療團隊之持續推動。

The population over 65 years old is expected to climb rapidly to 1.2 million in 2031,and this highlights the growing need for implementing holistic health care and strengthening the quality of in-home health services. However, the role and the responsibility of the Medical Laboratory Scientist (MLS) in the in-home care have yet to be determined. During March 1 to the end of July 2016, the MLS team of the Taipei JMS hospital participated in 30 in-home care services and 8 cases of Hospice Care. Services provided included blood test (66.7%), glucose detection (6.7%), and electrocardiogram examination (3.3%).The remaining tasks spanned from blood pressure measurement, solicitude to the documentation of medical records and relevant information. Our visiting experience shows that blood test is the first priority of the in-home care medical laboratory tests, with cholesterol, liver and kidney function, and routine blood check being the key parameters. We plan to further introduce portable electrocardiogram test, counseling, rapid screening and POCT (Point of Care testing) in the future according to the service demands. Rapid screening and POCT enable us to obtain the test results within 30 minutes. MLS team performs standard maintenance regularly on these devices to ensure the test quality, and trains other medical staff the correct use of the devices to enhance the synergy momentum of the in-home care medical team.
