縱使台灣全民健保已達到100% 的覆蓋率,但並不表示台灣的醫療就是成功的! 麥可.波特(Michael Porter)提出「病人價值」應是每一個國家健康照護體系改革中的基礎建設。從病人的角度審視護理的價值(Value-based care),期望在合理的成本下,為達到預期結果所能夠獲得安全、妥適,及有效益的照護; 從專業角度審視「價值」,應以實證醫療為依據,證實治療與處置的成效,同時考量病人的期望與偏好。開創護理價值是一個精進的過程,護理價值在臨床應用過程中須將低價值護理轉變為高價值護理,護理價值的轉變需要勇於改變思維、變換態度、重建行為去翻轉與創新護理的新思維。重點在於,護理領導人更需有效率及有影響力的引領變革,積極提升新世代護理人員的專業核心能力。另護理師必須從照護提供者,轉為照護成果當責者,才能重塑高價值護理照護,成功扮演民眾健康之守護者,奠定護理專業之社會價值。
It does not necessary mean a successful health care even if the national health insurance has reached a 100% coverage in Taiwan. Michael Porter (2010) pointed out that "patient value" should be a basic infrastructure within the reform of the health care system in each country [1]. From the patients' perspective, value-based care means a safe, appropriate, and effective care with enduring results; from the professional point of view, "valuebased nursing" means employing evidence-based medicine and proven treatments and techniques and in the same time, concern patients' wishes and preferences. Creating value-based nursing care is a sophisticated process in which the nursing value in clinical application is transformed from low- to highvalue care. Courage is required to change ones' thinking, attitudes and innovative practice on nursing. More importantly, the nursing leaders need lead the transformation efficiently and influentially, and actively improve the professional core competencies of the new generation nursing staff. Also, only when the nurses change from care providers to persons responsible for the outcomes that the high-value nursing care can be reshaped, play the guardians of public health successfully, and establish the social value of the nursing professionals.
