物理治療師參與出院準備服務居家訪視之經驗分享
The Experiences of Pre-Hospital Discharge Home Visits by Physical Therapists
摘要
良好的出院準備服務可銜接急性醫療與慢性照護，確保病患出院後獲得持續性照護，並促使其重返日常生活與社會角色。以往出院準備只限於會議形式，缺乏實地探訪居家環境，致使專業人員提供的建議與現實有所落差，藉由出院前提供物理治療居家訪視，更能確實評估病患在居家環境中呈現的真正能力，並依此給予更適切的日常活動注意事項、輔具及環境改造等建議，改善其生活品質。臺北市立聯合醫院秉持以人為本的精神，致力於改善出院準備服務的品質與流程，率先執行出院前物理治療居家訪視，除了病患、家屬獲益良多，更能突顯物理治療專業在出院準備中的重要性。建議未來應將出院前物理治療居家訪視納入出院準備計畫例行內容，讓更多病患及家屬受惠。

Discharge planning plays an important role in transiting acute medical care to chronic one, ensuring care continuity, and restoring patient’s daily living and social activities. In the past, pre-discharge planning was hold in hospital in a meeting form. Predischarge home visiting is an effective strategy to access patient’s exact ability in home environment and the professionals can then advise on precautious issues, use of assistive devices and modification of home space accordingly. With the commitment of humanism, the Taipei City Hospital dedicates to ameliorate the quality and the process of discharge planning, and takes the lead to provide pre-discharge home visit for future home physical therapy. This benefits not only the patients and their families but also highlights the importance of physical therapy in discharge planning. We recommend to include predischarge home visit before further home physical therapy should be included in the discharge planning service to benefit more people in the future.
