肺癌病人之中西醫護理經驗─個案報告

個案為62歲女性診斷為肺癌，有肺結核及地中海型貧血病史，因肺癌轉移併發肋膜積水而住院。護理期間自2017年8月22日至2017年11月30日，運用Gordon十一項健康功能評估，確立個案有營養不均衡、低效性呼吸型態、現存皮膚完整性受損健康問題。護理師發揮「以人為主」照護理念，協調出院準備專業團隊介入，包括中西醫師、復健師、藥師、營養師、社區護理師共同協助個案，同時強化照顧者角色功能，指導個案與外傭執行健生操、教導穴位按摩、傷口照顧、正確飲食及藥膳，中醫師以中醫及針灸治療，輔以穴位圖卡及穴位影片教導，有效改善個案健康問題。同時介入出院準備團隊以中西醫整合照顧模式，擬定照顧目標，讓個案及家屬能共享決策，安心放心出院回家進而提升生活品質。

A 62-year-old lung cancer female patient with the history of tuberculosis and thalassemia was hospitalized due to complications of pleural effusion caused by lung cancer metastasis. During August 22 to November 30 in 2017, we confirmed that she had an imbalanced nutrition, ineffective breathing pattern, and impaired skin integrity by using the Gordon 11 Functional Health Patterns. The nurses played a vital role in coordinating professional team's discharge planning for the patient and enhancing the function of the caregivers; the patients and foreign caregivers were instructed for doing healthy exercises, acupoint massage, wound care, and having healthy diet and herbal cuisine; also illustrations and videos were used to assist improving patient health problems efficiently. The discharge planning team used a Chinese-Western medicine integrated care model and set up a caring goal for the patient; the patient and her family members were invited to make shared-decision so that she could return home without worries and live a better life.
