以價值為基礎之職能治療照護

職能治療高價值照護是藉由主動參與出院準備服務，提供職能治療專業評估供個案、家屬及其他專業領域出院時之參考；此外對於有積極復健潛能個案提供住院期間急性後期高強度職能復健治療及出院後數週居家職能復健服務，藉密集復健療程由醫院延續到社區居家，把握積極治療期，縮短復健療程，提升個案日常生活功能與自我照顧能力，幫助使其儘早回歸社區生活，同時避免再住院及節省醫療資源；期間協助轉銜長照系統，善用長照資源，讓社區各項居家服務能夠延續。由於職能治療是本著以個案為中心思維積極介入，再加上個案主動參與，使得個案在接受職能治療後在生理、心理等層面上得到滿意的結果，藉此創造高價值職能治療照護。

The value of occupational therapy intervention can be best maximized by the proactive participation of an occupational therapist at the discharging planning. Patients, the families and other health care professionals can all benefit from the rehabilitation consultation prior to hospital discharge. In addition, high intensity post- acute rehabilitation can be offered to patients who have potential to recover rapidly during hospital stay, then transit to home-based rehabilitation program after discharge for the following weeks. This intensified rehabilitation course extending from hospital to patient's home can fulfill the need in subacute recovery stage, shorten the duration of rehabilitation, improve patient's ability to self care and enhance the performance of activities of daily life; as a result, patients may return to normal community life as early as possible, shorten hospital stay and reduce health care expenses. In some cases, patient can be referred to long term care facilities, keep utilizing long term care resources and extending medical care in the facility. The focus of occupational therapy care is patient-centered involving active participation, helping patients to achieve a satisfactory recovery both physically and mentally, in order to achieve high value occupational therapy care.
