摘要

目的：探討負壓隔離病房護理人員照護COVID-19所面對之衝擊、壓力以及運用調適機轉之心路歷程。方法：本研究採用質性研究法，以六項半結構式的訪談提綱作為訪談之指引並以深度訪談方式收集資料，立意取樣訪談8位於負壓隔離病房執行臨床照顧之護理師。結果：護理人員在照護嚴重特殊傳染性肺炎之心路歷程歸納出六大主題：（一）負重前行的感慨：工作壓力、倦怠感與正向思考；（二）擔心隔離之心聲：疾病的推手與朋友、家人的支持；（三）照顧病人的負擔：病人的情緒大於病毒的威脅；（四）依靠個人防護設備來確保安全：安心與不安心及內心壓力；（五）醫療團隊的互動：萬般無奈與支持系統；（六）自我調適來抗戰：增加自我知能與心理健康調適。結論：受訪者能正向因應護理人員壓力衝擊，透過提供完整適切的防疫教育訓練、給予足夠的裝備物資，有助於護理人員壓力、心理困擾的減輕。透過本研究結果，亦可提供醫院行政管理者與護理專業團體制訂相關政策之參考。

Objective: To investigate the mental adjustment and experience of the nurses who cared for the COVID 19 patients when confronting the impact and stress. Methods: This study adopted a qualitative design and used a 6-item semi-structured content to guide the in-deep interview. Eight registered professional nurses at the negative isolation ward were sampled and interviewed. Results: We inducted six themes of mental experiences from the nurses: (1) Heavy burdened: work stress, fatigue, and positive thinking; (2) Worry of being isolated: the disease pushing-hand and supports from friends and family; (3) workload of caring patients: more from the emotions than the virus threats; (4) ensure safety by personal protection equipment: Feeling safe and unsafe, and internal stress; (5) interaction of medical team: helpless and support system; (6) Self-adjustment to fight the war: enhance self-literacy and adjustment for psychological health. Conclusion: The participants can tackle the stress impact positively. Providing complete and adequate epidemic prevention education and enough personal protective equipment helps nurses relieve stress and psychological disturbances. This study offers related references for hospital administrators and professional nursing organizations when making related policies.
