精神科急診家庭暴力病患之特徵

Characteristics of Psychiatric Emergency Patients Who Committed Domestic Violence

摘要


目的:本研究旨在探討精神科急診就醫家暴病人之現況,並分析影響病人住院治療之相關因素。方法:本研究採病歷回顧分析方法,調閱2019年於某一精神科專科醫院急診就醫時被通報家暴之病人病歷進行分析。結果:本研究共收集291位病人資料,九成以上皆為家暴相對人,近九成都涉及肢體暴力。案件類型以成人保護案件最多,八成五以上的病人與家屬同住;近一半的家屬為病人的父母。病人年紀、有重大傷病卡、主要精神科診斷、精神病症狀、是否規律就醫、是否規則服藥、過去一年曾住院、兩造關係、家屬有精神疾病,皆與病人本次住院有顯著相關。病人有明顯精神病症狀、過去一年曾住院、主要診斷是自閉症或酒精使用疾患,最能預測病人的住院原因。結論:本研究結果顯示精神病人家暴樣態之特殊性。病人的臨床特徵為主要影響其住院治療的因素;病人的家庭支持系統亦值得關注。

Objective: The present study aimed to examine the characteristics of psychiatric patients with domestic violence in a psychiatric emergency room and factors affecting subsequent admission. Methods: Medical charts of patients notified of domestic violence when admitted to a psychiatric emergency room in northern Taiwan during 2019 were reviewed retrospectively. Results: The current study recruited 291 patients. Over 90% of the patients were domestic violence perpetrators, and nearly 90% of the cases involved physical violence. Most of the type of cases was adult protection. Over 85% of patients cohabited with their families. About half of patients' relatives were their parents. Patients' age, having major illness card, primary psychiatric diagnosis, psychosis, regularity of therapy attendance, medication compliance, hospitalization in the past year, the relationship between patients and their relatives, and relatives having a mental illness were all significantly related to current patients' admission. Apparent psychosis, hospitalization in the past year, and primary diagnosis of autism or alcohol use disorders predominantly predicted the admission. Conclusions: The results demonstrated the distinctiveness of domestic violence in psychiatric patients. Patients' clinical characteristics are primary factors affecting disposition decisions. The family support system merits additional attention as well.
