靜態電腦輔助設計與電腦輔助製造手術導引板於局部缺牙患者植牙手術的應用

摘要


靜態電腦輔助手術導引板的設計,是於軟體中結合事先設計贋復物的位置及預計人工牙根放置的角度與方向,需要仰賴牙醫師與牙技師的默契溝通及通力合作,並以電腦輔助製造方式產出。與植牙手術搭配傳統方式製作手術導引板相較,設計良好的靜態電腦輔助手術導引板可縮短手術時間,若搭配無翻瓣植牙手術,可以減少手術失血量、更可有效減少病人的術後不適感,且牙科植體的存活率與邊緣骨喪失並無不同。隨著植牙手術搭配靜態電腦輔助手術導引板的幾年來的經驗累積,將目前術前計畫植體位置與術後植體位置比對發現,準確度已顯著提升。應用數位流程,使用數位流程靜態電腦輔助設計/電腦輔助製造手術導引板將會是未來趨勢。

Through thorough communication between the dentist and dental technician, the static computer-aided designing and computer-aided manufacturing (CAD/CAM) of surgical guide involve combining the information of the premade prosthesis' position and expected implant angulation using software and produced with the aid of a computer. Compared with implant surgery using a surgical guide fabricated by conventional methods, implant surgery with CAD/CAM surgical guide shortens the surgery time. Implant surgery with CAD/CAM surgical guides combined with flapless surgery decreases the blood loss and post-operative discomfort significantly, with no difference between dental implant survival and marginal bone loss. Along with the accumulated experiences in implant surgery with CAD/CAM guide, the accuracy of implant position has increased, as evident from the comparison of pre-surgical and post-surgical implant location. Thus, the use of digital workflow and CAD/CAM surgical guides is becoming a future trend.
