評估不可磨粉藥品對簡易懸濁法的適用性

Evaluation of Applicability of Simple Suspension Method in Non-crushable Drugs

摘要


目的:評估院內不可磨粉的藥品是否可以以簡易懸濁法而崩解懸浮。方法:本研究期間為2020年7月至2021年5月收集某醫院院內不可管灌的藥品,且排除公費、管制藥品、臨採以及單價(健保價)高於100元/顆藥品。每項藥品皆準備3顆進行(一)崩解懸浮測試(Decay-suspension test)。(二)鼻胃管通過測試(Test for tube passage)。所有測試皆於室溫(25.8±0.48°C)、相對恆定濕度(64.0±6.98%),以及照度(1070.4±49.5 Lux)下進行。最後將適用於簡易懸濁法之院內不可磨粉藥品進行分析,可改善不適合磨粉藥品卻磨粉給管灌病人使用的機率。結果:本院口服藥品總共有560項,當中不適合磨粉藥品有169項,經排除納入之藥品,最後納入本研究總共有44項藥品進行簡易懸濁法的測試,研究發現44項不可磨粉的藥品中,有14項(31.8%)藥品適用簡易懸濁法,此14項藥品,有13項(除Atozet外)於室溫放置5分鐘內震搖即可完全崩解。結論:此研究是針對不可磨粉的藥品,但因藥品本身的特性,成功率會偏低,但仍有31.8%藥品適用簡易懸濁法,並可降低不適合磨粉藥品卻磨粉給管灌病人使用的機率。

Objective: To evaluate whether non-crushable drugs available in our hospital can be disintegrated and suspended using a simple suspension method. Methods: Non-crushable drugs of our hospital, excluding publicly funded, controlled, and casually procured drugs, as well as those with a unit price (health insurance price) >100 dollars, were collected from July 2020 to May 2021. Three tablets of each drug were prepared for decay-suspension and tube passage tests. All tests were performed at room temperature (25.8 ± 0.48°C), and at consistent relative humidity (64.0 ± 6.98%) and illumination (1070.4 ± 49.5 Lux). We aimed to identify non-crushable drugs that suit the simple suspension method to avoid administering non-suspensible drugs via nasogastric tube feeding. Results: There were 560 oral drugs in our hospital. Among the 169 non-crushable drugs, 44 met the inclusion criteria for a simple suspension experiment. Among the 44 non-crushable drugs, 14 (31.8%) were suitable for the simple suspension method in which 13 (except Atozet) disintegrate after shaking at room temperature for 5 min. Conclusion: This study considered the suitability of suspending non-crushable drugs using a simple suspension method. The suspension success rate was relatively low due to the drug characteristics. Still, the simple suspension method suits 31.8% of the drugs, thus reducing the chance of grinding and nasogastric tube feeding of the drugs unsuitable for suspension.
