矯正機關之海洛因成癮病犯接受美沙冬替代療法之臨床意涵－初探
Treatment for Heroin Addicts in Jail-A Preliminary Report
摘要
藥物濫用及毒品的危害，是全世界各國面臨的共同問題，全球估計約有1,300萬名靜脈注射藥癮者，其中約有400萬人已感染愛滋病，我國自民國76年通報第一例藥癮者共用針具而感染愛滋之個案，歷年藥癮者感染愛滋人數，僅為零星個案，惟自94年藥癮者感染愛滋個案人數遽增，人數高達2,461人，佔當年度所有通報個案72%，迫於藥癮者感染愛滋疫情不斷攀升，衛生署疾病管制局於94年起即積極推動減害計畫。所謂減害計畫(Harm Reduction Program)是一個全方位的整合性計畫，目的是希望降低毒品對個人、家庭、社會的傷害。 為使矯正機關內之藥癮收容人，出監所後能夠有效銜接替代治療，由屏東縣毒品危害防治中心協調屏東看守所與執行美沙冬替代療法之醫療團隊，自民國97 年4 月起，對於因違反毒品危害防治條例之海洛因成癮病犯進行評估，由醫療團隊每兩週進入看守所與即將服刑或勒戒期滿之個案進行訪談，並記錄其成癮史、戒癮史、前科次數、家庭社會職業功能、再犯可能性及接受美沙冬替代療法之意願等，將符合治療條件之收容人列入追蹤管理，並於出監所後能夠順利進入醫療體系接受治療。

Many countries are facing serious problems with illegal drugs. There are about 13 million drug users all over the world in which 4 millions are HIV infected. In our country, the first injecting drug user with HIV was reported in MK76. In the past years, injecting drug user with HIV was sporadically reported but case numbers increased rapidly in MK94 with 2461 cases reported in that year. Harm reduction program was delivered under CDC of DOH. The harm reduction program is an integrated plans to reduce the damage to people family & community as a result of drug usage. In order to provide more available methadone maintenance treatment to heroin addicts after releasing from jail, biweekly interviews for collecting information including addiction history、abstinence history、past criminal history、family & social-occupiational function、 relapse predicting & motivation for entering methadone maintenance treatment under the support of Pingtung drug abuse control center & prison. We select some people & follow-up after releasing from jail for joining further medical abstinence.
