運用Watson關懷理論照護一位腦性麻痺病童及主要照顧者之經驗

Applying Watson Caring Theory to Assess a Child with Cerebral Palsy and the Caregiver Nursing Experience

摘要


本文係探討一位腦性麻痺病童及主要照顧者之護理經驗,護理期間自97年11月15日至12月1日,利用個案身體評估、觀察及會談的方式進行整體性的護理評估,以了解腦性麻痺病童常見的問題及其主要照顧者所面臨的壓力及需求。透過評估結果顯示主要護理問題,在個案方面有:潛在性危險性肺吸入/與無法吞嚥有關、呼吸道清除功能失效/與病童無法自咳有關;在主要照顧者方面:照顧者角色緊張/缺乏照顧的技巧及自信有關、家庭因應失能/家屬間產生極大分歧。筆者運用Watson關懷理論作為護理過程中的照護指引及護理措施之依據,以維持病童基本生理功能、緩解主要照顧者的困惑、壓力與情緒,並獲得家人的支持,除個案還包括其主要照護者的身、心、靈及社會層面受到更完整的照護,已達全人照護基礎,更落實以家庭為中心的護理。

This article evaluate the nursing experience of a caregiver and a child with cerebral palsy from November 15(superscript th) to December 1, 2008, using physical assessment, observation and a series of talk to assess the most common problem of a child with cerebral palsy and the pressure and need of the nursing caregiver. The assessment result showed that in this case the risk of aspiration of the child with cerebral palsy was related to inability to swallow due to ineffective airway clearance and inability to cough oneself., while on the part of the nursing caregiver, the difficulty and strain of taking care of the patient, the feeling of incompetency and anxiousness, the family coping ability to the situation, and the great differences between the family member. The author used the guideline and nursing intervention of Watson caring theory to maintain in a child with cerebral palsy basic physical function. Reliving the caregiver's confusion and stress, giving emotional support, achieving family support, as we as to achieving the goal of giving the child with cerebral palsy a complete care, including physical, psychological, spiritual and social. Reaching the nursing basic foundation and implementation of a family-centered care.
