運用安寧療護理念於一位肝細胞癌末期病患之護理經驗
Hospice Care Nursing Experience Related to a Patient with Terminal Stage Hepatocellular Carcinoma
摘要
本文係描述照護一位肝細胞癌個案，曾多次入院接受經導管動脈化學栓塞治療及經皮肝腫瘤內酒精注射治療，此次因更換膽管管路而入院，住院期間發現合併肺轉移之護理經驗。護理期間為2009年10月26日至2010年1月4日，筆者運用Gordon十一項健康功能評估，藉由觀察、會談及身體評估等技巧，收集個案相關資料，整理分析後確認個案有死亡的焦慮、慢性疼痛及低效性呼吸型態改變等主要健康問題。在照護期間，透過芳香療法、穴位指壓、傾聽、引導及生命回顧等護理措施，幫助病患緩解因生理症狀所帶來的不適，並協助個案及其家屬面對疾病病程快速惡化與接受疾病末期的事實，進而引導個案訴說心中、擔心放不下的事情，藉由陪伴、傾聽與關懷，讓個案可以泰然地面對死亡，體會生命意義，走出死蔭的幽谷。

This article describes a nursing experience of a patient suffering from hepatocellular carcinoma, which has been in hospital for several times to receive Transcatheter Hepatic Arterial Chemoembolization and Percutaneous Ethanol Injection, and this time was to change Percutaneous Transhepatic Cholangiography and Drainage. The nursing period was from October 26 in 2009 to January 4 in 2010. The author employed Gordon's functional health pattern model to collect the relevant information through observation, conversation and body assessment. Three major healthy problems were established as death anxiety, chronic pain and the change of ineffective breathing pattern. During nursing period, the author applied aromatherapy, acupressure massage, attentively listening, guiding and life review to help the patient to manage the uncomfortableness of physiological symptom and assist the patient and family to accept and face the progression of the disease and then guide the patient to share the worriment in mind. The patient can face death peacefully, understand the meaning of life and walk out of the dismal by accompanying, listening and caring.
