一位急性心肌梗塞併發肺水腫患者之護理經驗

本文敘述一位壯年的男性，因罹患急性心肌梗塞併發肺水腫住入加護病房之護理經驗。護理期間自2010年5月18日至5月25日，過程中藉由臨床護理、觀察會談、查閱病歷及追蹤訪談方式收集資料，並運用Gordon十一項功能性健康型態評估，確立個案有「氣體交換障礙」、「焦慮」、「健康尋求行為」護理問題。照護期間，密切監測生命徵象、意識狀態、觀察組織缺氧徵象，教導有效深呼吸咳嗽方法，提升肺部通氣功能，以改善氣體交換障礙；予個案介紹環境和解釋監測儀器，並主動關懷引導表達其內心感受和疑問，安排醫師解釋說明疾病治療處置、預後及注意事項，減輕病患心中疑惑，降低疾病帶來的不適和焦慮；提供飲食運動衛教與教導調整生活作息，強調戒菸戒酒的重要性，提升對疾病日常自我照顧知識，預防疾病再度侵襲。

This article describes the nursing experience of caring a middle-aged male who admitted to the Intensive Care Unit due to the onset of acute myocardial infarction complicated with pulmonary edema. During the nursing period between 18th May and 25th May, 2010, data was obtained by observational conversation, chart review and follow-up interviews. Gordon's Functional Health Patterns was used to assess the problems associated with the patient. The identified problems consisted of impaired gas exchange, anxiety and health seeking behaviors. Strategies for improving care included close monitoring of vital signs, state of consciousness and signs of tissue hypoxia. The patient was taught about effective coughing and deep breathing to improve pulmonary function and gas exchange impairment. He was helped to get acquainted with the new environment and monitoring instruments, and enabled to express feelings and ask questions after provided with initiative care. Meetings were arranged with physicians to discuss treatment intervention, prognosis, and precautions for his illness to reduce his worry, discomfort and anxiety arising from the disease. Health education for normal diet and exercise was provided to advise him a healthier lifestyle, emphasize the importance of tobacco and alcohol cessation, enhance the knowledge on daily self-care, and eventually to prevent subsequent disease recurrence.
