Distal humerus fractures clinically are not common, but they remain some of the most challenging injuries to manage. They are commonly multifragmented, occur in osteopenic bone, and have complex anatomy with limited options for internal fixation. Treatment outcomes are often associated with elbow stiffness, weakness and pain. To achieve a painless, stable, and mobile elbow joint, pre-operative evaluation should be performed thoroughly including medical condition, the soft tissues' condition, selecting a surgical approach, and selecting an implant for fracture fixation. Our case, a patient with a history of left humeral shaft fracture, suffered from comminted fracture of left distal humerus complicated with broken intramedullary nail. We performed internal fixation with Y plate for this patient and she recovered well with bony union. This result revealed that the Y plate can give a good outcome to a patient with distal humerus fracture.
