照顧一位吸食K它命導致間質性膀胱炎患者之照護經驗
A Nursing Experience: Looking after a Patient with Interstitial Cystitis Caused by Katamine Abuse
摘要
本文主要探討一位27歲男性因長期服用K它命而引發間質性膀胱炎之護理經驗。筆者於2010年1月25日至2月7日照護個案期間，運用生、心、社會、靈性層次為評估工具，藉由直接護理、觀察、會談、及身體評估等方式收集相關資料，確立個案的健康問題有：疼痛、個人因應能力失調等，並擬定個別性護理計畫。筆者在護理過程中，先衛教個案運用疼痛控制技巧及學習緩解疼痛之復健運動，緩解生理上不適感外，特別關注是否持續使用K它命而降低膀胱灌注的效果，再藉由提供疾病相關資訊，進而增進生活的滿意，避免因疾病相關資訊不足而引起返家後自我照顧的不確定感，導致反覆出入院，並引導抒發焦慮之感受，透過個案及醫療團隊之共同努力，積極發展個案因應能力及堅定戒毒的決心，協助建立自信與自尊，重新正常生活，期望藉由此護理經驗，提供日後照護此類病患之參考。

The article presents a nursing experience on a 27-years-old male suffering from interstitial cystitis, due to 5-years Katamine abuse. From January 25th to February 7th, 2010, the Author looked after a patient, and collected data from physical, psychological, social, and religious aspects through direct nursing, observation, discussion, physical appraisal, and so on.Eventually we found below symptoms in the patient: Pain, Coping ineffective individual. Accordingly the author drew up an individual nursing plan.In the nursing process taught the patient pain control skills during recovery and rehabilitation exercise to reduce pain. Besides alleviation of physical pain, we also observed whether patient continuously took Katamine and reduce the effect of urinary bladder irrigation. Further, we provided the patient with disease related information and trained him the ability of self-care in order to increase life satisfaction and reduce anxiety after returning home caused by insufficient information, which might result in recurrent hospitalization.By cooperation of patient and medical team, patient developed determination to quit drug abuse, established self-confidence & self-esteem, and regained normal life. We present this experience as a reference for any other cases of the same kind in the future.
