視神經脊髓炎病人成功脫離呼吸器之護理經驗
Nursing Experience of Ventilator Weaning for a Patient with Neuromyelitis Optica
摘要
本文探討一位30歲女性，剖腹產後引發視神經脊髓炎合併呼吸衰竭，成功脫離呼吸器之護理經驗。護理期間自2013年6月6日至6月21日，透過身體評估、直接照護觀察、傾聽、會談、團隊討論及查閱病歷收集資料，運用Gordon十一項功能性健康型態評估，確立健康問題為呼吸道清除功能失效、焦慮、身體活動功能障礙及家庭因應系統失調。照護過程中建立良好護病關係及信任感，運用護理專業知識並參考國內外相關文獻，掌握疾病特性，提供個別性護理措施，協助病人積極復健，增進肌肉力量，維持身體功能，漸進脫離呼吸器，同時提供疾病相關資料，指導家屬後續照護技巧，提升自我照顧能力，最終順利轉至普通病房後出院。

This article reported a successful nursing experience in ventilator weaning of a 30-year-old female patient, who was diagnosed with neuromyelitis optica soon after caesarean section for twins. The patient was cared at time from June 6 to 21, 2013. Comprehensive assessment was applied with Gordon's eleven functional health patterns by means of direct care, observation, conversation, physical examination, and chart review. Major health problems were identified as: ineffective airway clearance, anxiety, impaired physical mobility and family coping ineffectiveness. During the caring process, the author established trust and good relationship with patient and families, applied clinical nursing expertise to provide individualized interventions, helped the patient wean from ventilator through active rehabilitation and muscle power promotion. Meanwhile, the author also provided diseases related information and long term care resources, taught patient and families skills for further care, enhanced self-care ability, and facilitated a successful transfer to a general ward and discharge afterwards.
