This 35 years old nurse who suffered from weakness of the right limb due to hemorrhage in the left basal nucleus. We used Gordon's eleven functional health patterns as assessment tools from November 30 to December 21, 2017. Collect datas through listening, observation and physical assessment to establish that the case had impaired nutritional status, lack of self-care ability and hopelessness of health problems. Using individualized nursing plans to guide the life style of individual cases and seriously carry out the risk of disease management and rehabilitation, strengthen self-care ability, increase self life control to reduce the impact of physical, psychological and social roles. Through this care process, we found that the young and middle-aged population neglected the prevention of acute stroke. According to the concept of prevention is better than cure, the risk management of hemorrhagic stroke and chronic diseases in young and middle-aged people will be improved.
