Influencing Factors of Quality of Life in Hand Injury Patients - A Systematic Review of Literature

摘要

目的：探討手創傷術後患者生活品質改變的影響因素。方法：搜尋中英文電子資料庫收錄2010至2018年期間發表之文章，依納入、排除條件篩選，並依據實證等級評讀。結果：共7篇文獻，多為青壯年工作受傷。手創傷導致疾病壓力、疼痛、外觀改變、手功能障礙、參與社交意願低、日常與工作能力受限、焦慮及低自信心等問題，均對生活品質產生影響。手損傷愈嚴重，生活品質滿意度愈低；但隨著手功能逐漸恢復，生活品質滿意度亦趨提升，在受傷後3個月至1年期間更為顯著。結論：手創傷術後有功能障礙、外觀改變、低自信心、日常活動及工作能力、社會參與低等問題，因此手創傷術後生活品質應該被追蹤，讓醫護人員瞭解患者健康問題與限制，進而提供合適的照護，增進其生活品質。

Purpose: To investigate the possible influencing factors of quality of life (QoL) in patients with hand injuries. Method: The literature was reviewed on 5 databases, dating during 2010 to 2018, according to keywords and screening of inclusive and exclusive criteria. Classification of evidence adapted from Oxford Center for evidence-based medicine for empirical medicine. Results: Totally 7 eligible studies were selected. Influencing factors of QoL were classified as demographic, physical, social and psychology domain respectively. Severity of injury, pain, appearance change, disability, social function and negative mental status can influence QoL. Patients may require 3-12 months' recovery program to improve hand function and QoL gradually. Conclusion: Early identification of all influencing factors of QoL in hand injury patient and determination of the appropriate timing for health interventions will be able to explore illness cognition and related QoL in patient with hand injury, may improve of QoL in the future.
