照護一位新診斷愛滋病個案之經驗

The Nursing Experience of a Patient with Newly Diagnosed Acquired Immunodeficiency Syndrome

摘要

本文為一位新診斷愛滋病感染之護理經驗，個案因肛門廔管至大腸直腸科門診治療，檢驗發現感染愛滋病毒轉介感染科，筆者於2019/03/12至2019/04/30護理期間透過個案回診時評估發現個案因服用雞尾酒藥物引起急性疼痛副作用、對愛滋病特定知識缺乏及罹病後對未來感到無助等心理問題。個案初診斷時情緒不安，筆者藉由主動關懷、直接會談以及社交軟體溝通等方式與個案建立良好治療性關係，願意表達其內心真正需求與無望感，協同醫療團隊與伴侶討論個別化治療計畫及護理措施，讓個案與伴侶共同面對衝擊，順利渡過疾病過程，學習自我照顧，改善服藥不適、安全性行為及疾病之接受度等問題。期望此報告日後可以提供醫療照護人員運用不同的思維去處理並且幫助更多愛滋個案，提昇護理照護品質。

This case report was to describe a nursing experience of a patient with newly diagnosed AIDS. The case was treated at the colorectal clinic due to anal fistula, and then the HIV infection was found and referred to the infection department. The nursing period was from March 12 to April 30, 2019. The author assessed the case during the medical follow up, and found that the case had acute pain and side effects of cocktail treatment, lack of knowledge about AIDS, and feeling helpless for the future after the illness. The patient suffered from the negative emotion at the initial diagnosis. The author established a good therapeutic relationship with the patient through caring, by direct interview and social software communication. The patient was willing to express his true inner needs and feelings of hopelessness, and collaborated with the medical team to discuss individualized treatment plans with the partner. The case and the partner could face the impact together, and go through the disease process smoothly for learning self-care, and improving the discomfort of taking medication, safe sex, and the acceptance of diseases. The experience could be the reference of providing care to patients with AIDS by different thinking and improving the quality of nursing care in the future.
