Cancer has been the leading cause of death for 24 years in Taiwan. To ensure cancer patients admitted in acute care wards can receive hospice service, Bureau of Health Promotion, Department of Health, R.O.C initiated a hospice shared care project. It is aimed to integrate palliative and primary care teams to care terminal cancer patients and family in a share care model, which incorporates care of physical, psychosocial and spiritual issues. This article is to introduce development of hospice shared care programs, role and function of team members as well as evaluation of success. The difficulty, challenges and possible resolutions in practicing shared care are also discussed. We wish it could be an important reference to clinical practitioner for developing individualized care and service model.
