The Effectiveness of Implementing Advance Care Planning for the Elderly Living in Long-Term Care Facilities: Systematic Review
執行預立醫療照護計畫措施於長照機構老人之成效：系統性文獻回顧
摘要
目的：以系統文獻回顧了解預立醫療指示計畫措施執行於長期照護機構老人現況。設計：本文所採用的系統性文獻回顧方式是以Cochrane回顧為準則。方法：本文所使用的搜尋策略包括灰色文獻與以下電子資料庫：MEDLINE, CCTR, CENTRAL, CINAHL, PubMed （OVID），Academic Search Complete（ACP Club），PsycINFO/PsycATRTICLES（1991年1月至2012年12月）：中文的華藝資料（Airiti Library）（2000年5月至2012年12月）。此次回顧主要以隨機控制研究（randomize control trials, RCTs）、類實驗（quasi-experimental studies）或是具控制組（control group studies）的研究方法為收錄對象。結果：共收錄二篇隨機控制研究（RCTs）和一篇類實驗法的文章；結果顯示目前預立醫療指示計畫措施執行於長期照護機構老人的成效微弱。結論：此三篇研究因其預立醫療照護計畫的相關措施變異性差異太大且其執行措施的過程和內容也未被詳細說明，加上所應用措施的相關理論也並未被清楚呈現，導致措施成效薄弱。未來研究，針對措施所基於的理論以及所執行的措施內容需要詳細描述，以便增加研究所須涵蓋的資料。

Purpose: To systematically review evidence concerning about the effectiveness of implementing advance care planning for older people living in long-term care facilities. Design: Systematic review of literature based on Cochrane review criteria. Method: The grey literature and the following electronic databases were searched: MEDLINE, CCTR, CENTRAL, CINAHL, PubMed (OVID), Academic Search Complete (ACP Club), PsycINFO/ PsycATRTICLES (January 1991 to December 2012); Airiti Library (in Chinese) (May 2000 to December 2012). The review chiefly focused on randomized control trails, Quasi-experimental or control group studies. Findings: Two RCTs and one quasi-experimental study were included. The results of this review present weak evidence for the effectiveness of interventions to improve the implementation of ACP in long-term facilities. Conclusion: The effectiveness of interventions to execute ACP for older people living in long-term care facilities presented weak evidence because the components of the interventions were varied and were not documented in sufficient detail to enable assessment, and the theory in the design of interventions also had not be presented enough. For future researches, the theory in the design of interventions as well as the components of the interventions need to be described in detail to maximize the information contained in the research.
