外科專科護理師工作職能之研究－以亞東紀念醫院為例

摘要

近年來台灣因醫療院所床位的快速增加，然而由於醫師人力的培育受到政策限制，故人力供需不平衡之缺口，致使各醫院為解決醫師人力不足問題，而紛紛自行培育專科護理師，並使其在醫師的監督下協助執行醫療行為。但是究竟專科護理師應具備何種職能，才能使其勝任其工作，目前卻仍缺乏明確的專科護理師之職能內涵之研究，故本研究期能就此項課題加以探究，以界定出專科護理師之職能構面。本研究除彙整相關文獻及歸納出影響專科護理師每日工作之主要因素，並據此制定出專科護理師之職能構面初稿，並以專家會議來修定問卷。其次，採取立意抽樣方式，針對醫學中心的專科護理師，為問卷發放對象，並以加權平均數與層級分析法作資料之分析。本研究建構出專科護理師工作職能之架構，經以層級分析法分析後，第一層級四個評估準則，分別為技能、專業知識、個人特質、能力，第二層級整體評估因素計有十二項，第三層級整體評估因素計有三十六項。本研究之研究目的為建構出專科護理師應具備之核心職能特質構面之組成因素與建構出專科護理師之核心職能特質模式中，各子因素之權重排序，以區分各子因素間之重要性差異，以供作未來護理主管人力資源決策之參考。根據前述研究目的之描述，本研究之主要問題為探究專科護理師所應具備之工作職能構面與探究護理師工作職能構面各子因素之權重排序。

In current years, lots of hospitals breed nurse practitioners to play an important role in medical service under the doctor's clinical governance due to rapid increase in inpatients and insufficient physicians under the restriction of the government. However, what competency a nurse practitioner should acquire to handle his/her work still lack a full study of definite profile for their competency on job. This study focused on the nurse practitioners' competency at work, trying to define a constructional profile.This study generalized the major factors which affect the loading of nurse practitioners in daily work and made questionnaire modified by focus group meeting and prospective aspect. Then, by Convenience sampling distribution, these questionnaires were filled up by nurse chiefs, physicians and nurse practitioners of thirteen Hospitals Department o Health Executive.The study figured out the construction of the effect of the nurse practitioners with competency via the method of AHP-the first level consisted of 4 evaluating values, from the most important one, as skills, professional knowledge, personal constitution and ability; the second level consisted of 12 factors; the third level consisted of 36 factors.
