目的:本研究探究克氏行為量表(Clancy Behavior Scale, CBS)在基層診所與綜合醫院,篩檢自閉症類群障礙症(autism spectrum disorder, ASD)嬰幼兒的效度。方法:參與者為180位生理年齡介於16~30個月的嬰幼兒,包括ASD嬰幼兒、一般發展(typically developing, TD)嬰幼兒與發展遲緩(developmental delay, DD)嬰幼兒各60位,由母親填寫CBS。以訊號偵測理論決定CBS的切截分數。結果:研究結果發現CBS篩檢ASD與TD嬰幼兒時,以11分作為切截分數,敏感度約.88,特異度約.82;9題關鍵題目,以7分作為切截分數,敏感度約.87,特異度約.85。CBS篩檢ASD與DD嬰幼兒時,以13分作為切截分數,敏感度約.75,特異度約.73;8題關鍵題目,以7分作為切截分數,敏感度約.78,特異度約.72。結論:本研究結果支持CBS可以用來篩檢ASD嬰幼兒,特別是基層診所的早期篩檢。
Objective: Screening tools for autism spectrum disorder (ASD) are divided into two levels. Level 1 screening tools are used for screening in general populations, whereas Level 2 refers to those used for screening at-risk ASD sample. However, few studies examined the accuracy of early screening for ASD in Taiwan. The Clancy Behavior Scale (CBS) was used to detect autism spectrum disorder (ASD) in toddlers in general populations (e.g., primary clinics settings) and at-risk ASD sample (e.g., general hospitals settings) in this studies. Methods: There were 180 toddlers from 16 to 30-months-old, including 60 toddlers with ASD, 60 toddlers with typically developing (TD), and 60 toddlers with developmental delay (DD). Mothers of all toddlers were invited to fill out the CBS. Receiver operating characteristics was used to select a cutoff score for the CBS. Results: The score of 11 was used as cutoff to distinguish toddlers with ASD from toddlers with TD. The sensitivity and specificity were .88 and .82, respectively. Using the structure matrix of the discriminant analysis, we selected nine items from all 14 items of the CBS as critical items. The score of 7 was used as cutoff of the nine critical items to distinguish toddlers with ASD from toddlers with TD. The sensitivity and specificity were .87 and .85, respectively. In addition, the score of 13 was used as cutoff to distinguish toddlers with ASD from toddlers with DD. The sensitivity and specificity were .75 and .73, respectively. Using the structure matrix of the discriminant analysis, we selected eight items from all 14 items of the CBS as critical items. The score of 7 was used as cutoff of the eight critical items to distinguish toddlers with ASD from toddlers with DD. The sensitivity and specificity were .78 and .72, respectively. Conclusion: The results of this study suggested that the CBS could be used as a screening tool for toddlers with ASD less than 30-months-old, especially for primary clinics settings.
