提升加護病房護理人員交班滿意度之改善專案

目的藉由縮短交班時間、減少重複交班的案件數及增加交班完整性進而提升加護病房護理人員交班之滿意度。方法本單位經現況分析後發現，每床交班花費平均時間為6分53秒，重新交班案件的比例有43.92%，重新交班時每床花費平均時間為9分52秒，而且人員對於整體交班滿意程度只有68.89%。藉由制定「加護病房護理交班指引單」、訂定「加護病房護理交班指引單內容規範」、施行教育訓練、以及採固定段落照護方式排班。結果專案改善後，每床交班花費平均時間為3分31秒，重新交班案件的比例有19.80%，重新交班時每床花費平均時間為4分25秒，而且人員對於整體交班滿意程度提升至83.53%，皆達到專案的目的。結論藉由制定「加護病房護理交班指引單」、訂定「加護病房護理交班指引單內容規範」、施行教育訓練、以及採固定段落照護方式排班，可以增加護理人員交班之滿意度。

Purposes The aims of this project were to improve satisfaction with the nursing handover in the intensive care unit by shortening the handover time, reducing the number of repeat handovers, and increasing handover integrity. Methods Before this project began, the average time for handover per patient was 6 minutes and 53 seconds, the rate of full reports was 43.92%, the average time for full-report case handover was 9 minutes and 52 seconds, and the rate of nurses' satisfaction with the handover was only 68.89%. The project included designing a handover sheet for ICU nurses, establishing standard guidelines for the handover, carrying out an in-service education program, and limiting the number of patients cared for by each nurse so that the patients got continuous care. Results After the implementation of this project, the average time for handover per patient was reduced to 3 minutes and 31 seconds, the full-report case rate was lowered to 19.8%, the average time for full-report handover was reduced to 4 minutes and 25 seconds, and nurses' satisfaction with the handover increased to 83.53%. The aims of this project were achieved. Conclusions The project improved the level of satisfaction with nursing handover by designing a handover sheet for ICU nurses, establishing standard guidelines for the handover, carrying out an in-service education program, and limiting the number of patients cared for by each nurse so that each patient received continuous care.
