輻射屏蔽之有無對接受心導管診療病患皮膚劑量之比較

Comparison of Skin Dose with and without Radiation Shield to the Patients during Cardiac Catheterization Procedure

摘要


本研究的目的在於以侖道假體(Rando Phantom)代替病患模擬冠狀動脈介入性治療中的X光照射時間,分析病患在治療過程中接受的皮膚劑量。本研究根據新光醫院心導管室2005年病患服務量,依據治療型式分為四組,分別為(1)只做診斷,未作治療;(2)PCI一條血管;(3)PCI兩條血管;(4)PCI三條血管。統計每組接受X光透視和照像的時間之平均值作為模擬照射時間之依據;並以熱發光劑量計(GR-200F)在侖道假體佈點,佈點位置包括左右眼角處(眼睛)、頸部中心左右各1公分處(甲狀腺)、右胸乳頭下(胸腺)、生殖器上三公分處,及左胸心臟處皮膚表面以代表各敏感器官及基底層之皮膚劑量。將熱發光劑量計進行計讀,可得知各佈點器官之吸收劑量;分別乘以該器官之輻射加權因數後可得到有效劑量值。結果顯示,無輻射屏蔽之皮膚劑量分別為8.81,16.23,38.24,26.01cGy;有輻射屏蔽之皮膚劑量分別為7.55、14.90、36.12、24.43cGy和無屏蔽的皮膚劑量值比較分別降低了14.3%、8.2%、5.5%、6.1%。這些劑量數值皆在閾值2Gy以下,不會造成急性或慢性皮膚效應。

This study is aimed to measure patients' skin radiation dose during the PCI procedure with TLD and Rando phantom. The result is intended to offer concrete suggestion and valuable information to help the cardiologists to aware the potential radiation injury and take responsibility to reduce the patients' radiation dose. This study targeted on the service amount of cardiac catheterization laboratory of Shin Kong Hospital during 2005. Potential samplers were divided into four groups; the exposure time of each group varied. Rando phantom and TLD GR-200F chips were used in this study to measure the possible radiation exposure to the human body. The skin dose of four groups without radiation shield are 8.81, 16.23, 38.24, 26.01 cGy, respectively. The skin dose of four groups with radiation shield are 7.55, 14.90, 36.12, 24.43 cGy, respectively, the dose decreased 14.3%, 8.2%, 5.5%, 6.1%. According to the results, the chronic and acute skin effects did not occur.
