多葉式準直儀模型化參數的調整對劑量影響的探討

The Influence of Dose Calculation Affected by Offset Corrections of Multi Leaf Collimator Mechanical and Radiation Position in Radiation Therapy Treatment Planning System

摘要


強度調控放射治療儘管在放射治療界已經廣爲運用,多葉式準直儀系統、反向治療計畫,準直儀序列的執行,精密且方便的治療技術因應而生,而治療的準確必須仰賴一開始精準的接收測試及品質保證去持續進行才得以保障病患的治療品質。影響強度調控放射治療品質的來源不外乎是電腦治療計畫的計算偏差,測量工具選用不適當,或是執行照射時機器的輸出不穩定,或病患擺位所造成的誤差。其中,以電腦治療計畫爲誤差因素所佔比例最多,因爲在電腦治療計畫整體計算的程序之第一要素爲將直線加速器的臨床資料輸入電腦治療計畫中並完成接收測試,然後進行驗證劑量並應用於臨床的治療。本研究觀察在治療計畫內物理部分的模型參數對劑量的影響;並且在多葉式準直儀在電腦治療計畫系統的模型參數設定方面,針對葉片的弧形邊緣的葉片特性,經過射束投影到治療照射平面上時,葉片的邊緣位置必須同時考慮到兩種狀況,一是電腦治療計畫設定的葉片位置,此爲光照野的位置,另一個是輻射的物理劑量實際到達治療平面的位置,經由實驗的擺設與劑量驗證,我們試圖找出兩種位置的關係,並驗證在執行葉片的邊緣位置設定時,計算加速器要照射的監控單位(monitor unit)選取的位置落點必須在輻射劑量剖面曲線50%的位置,劑量計算後的結果不至於會超過或低於應給的處方劑量,藉本研究正確設定兩種位置的偏移量可解決在電腦治療計畫計算時,因光照野和劑量剖面曲線偏移量設定不佳所帶來的劑量傳遞誤差。

Intensity-modulated radiation therapy (IMRT) is a widely accepted method for radiation treatment to provide a prescribed and uniform dose to the target volume and a minimum dose to normal tissues that is dependent on the IMRT software and the treatment machine. Due to the possibility of escalation of the tumor dose by using IMRT technique, the accuracy of dose output from linear accelerator becomes the most import issue in routine quality assurance. There are sources of error in IMRT quality assurance and are probably in measurement process, treatment planning system, and delivery system. The most important error source is treatment-planning system. Beside the characterization of the MLC, particular attention should be paid also to the performances of the linear accelerator in extreme situations, like as the output point for the dose profile. It will affect the dose calculated in treatment planning system. The study investigate the treatment planning system ADAC pinnacle3 version 7.6 that there are two positions in MLC round leaf end. One is actual treatment position, and the other is leaf position in pinnacle3. How these two positions affected the dose delivery and after correction of the offset parameter to have a good fit between calculated and measurement dose of is the aim of this study.
